Arturo: Welcome to Altitude. Today we’re tackling a topic that’s as common as it is dangerous in aviation: crew fatigue.
Anyone who has been in a flight deck or managed crew schedules knows there are days when your body’s there, but your mind isn’t.
The problem is, this gets normalized. It’s seen as “part of the job.” But fatigue is not a minor inconvenience—it’s a critical operational risk, just as serious as a technical failure.
Joining me today is Nicolás Rhoads, a great colleague with a comprehensive perspective from regulatory, data analysis, and operational angles.
Nicolás: Thanks, Arturo. This is one of those issues everyone knows is a problem… but rarely gets addressed with the seriousness it deserves. Fatigue isn’t a “soft” issue. It has measurable effects on attention, judgment, memory, and coordination.
And when you’re in command of an aircraft, even minor degradation can mean the difference between a safe flight and an incident.
What Is Fatigue in Aviation? (4 min)
Nicolás: First, we need to understand that fatigue isn’t just being tired. It’s a physiological and cognitive condition.
There’s physical, mental, and emotional fatigue. It can stem from long duty days, interrupted sleep, circadian rhythm disruption (e.g., night shifts or time zone changes)… even chronic stress.
The tricky part is, fatigued individuals often don’t realize it. Studies show that even qualified pilots self-assessed as “fit to fly” while displaying cognitive impairment in objective tests.
Arturo: I’ve been through it many times. On long flights or complex rotations, you operate on instinct, on routine… almost like autopilot.
Nicolás: Like when you drive home and don’t even remember how you got thereyou’re distracted, thinking of a million things.
Arturo: And that works—until something goes off-script. That’s when it shows: you need sound judgment, anticipation, quick reactions. And if you’re fatigued, that’s just not there. Sometimes you don’t even remember the last few minutes of flight. Or you read the briefing and don’t process it.
The scary part is that these lapses fly “under the radar.”
Contributing Factors (5 min)
Nicolás: Here’s where it gets more complex. Fatigue doesn’t come just from flying too many hours. Irregular schedules, night shifts, red-eye flights, time zone changes… Or layover conditions: noisy hotels, long transfers, altitude changes.
Even time spent awake before the flight, long briefings, or waiting on the ramp can add up.
Arturo: And if you add emotional stress—family issues, financial problems, poor workload distribution—it becomes a dangerous cocktail.
I remember rotations where you had just 8 hours between flights… and that wasn’t real sleep time. It included transport, check-in, meals, and trying to sleep with your internal clock flipped.
Nicolás: There are also cultural factors: airlines where it’s all about “toughing it out,” “don’t complain,” “do your job.”
In that environment, who’s going to raise their hand and say, “I’m fatigued, I shouldn’t fly”?
Arturo: No one. Because doing so can be frowned upon—or even punished. That’s why systems must include safe ways to report fatigue without fear.
Notable Case Studies (4 min)
Nicolás: One of the hardest cases was Colgan Air 3407. The pilots weren’t just poorly rested—they had traveled by bus and slept in crew lounges.
Both had minimal rest in the previous 24 hours. And during approach, they mishandled a stall that they could’ve managed under better conditions.
50 people died. It was a turning point in U.S. fatigue policy.
Arturo: Another case: Air India Express 812. The captain, after insufficient rest, made multiple errors on approach to Mangalore.
He overshot the glide path, ignored alerts, and the aircraft overran the runway. The rest profile showed he was fatigued—even though he was deemed “fit.” That’s the scary part: validation systems don’t catch invisible deterioration.
And FRMS systems aren’t perfect—nor will they ever be. A rotation might be well-designed and approved, but if the crew doesn’t restwhether due to worry, visiting someone, or even partyingthat fatigue won’t be “logged,” but it’s still there.
Arturo: Someone asked us recently, “Are there regulations for this?” And let’s be clear: yes, there are.
Duty limits, hour caps—they exist. But who assesses the real quality of rest?
Nicolás: Exactly. Many operators comply on paper, but ignore the intent. They follow the numbers, but design rotations that, while “legal,” are unsustainable. Often, FRMS implementation is symbolic: you fill out the form, but there’s no real risk assessment using operational data, health metrics, or alert tracking.
Arturo: And if pilots don’t trust the system—or fear being punished—they won’t report fatigue. And without that trust, a true fatigue mitigation culture can’t exist.
What Can Be Done? (6–7 min)
Arturo: So Nico, you’ve managed crew scheduling for years—what can be done?
Nicolás: First, recognize that fatigue is inevitable—but manageable. We need a mindset shift: from legal compliance to risk-based thinking. Invest in FRMS systems that include physiological data, sleep behavior, chronotypes, etc.
Use tech tools—from fatigue monitors to risk simulations for flight schedules.
It sounds complex, but even small things matter—like choosing quality layover hotels.
Too often, companies pick the cheapest, forgetting that a hotel is a rest tool. It should have blackout curtains, 24-hour dining, soundproof rooms, etc.
Arturo: And let’s talk self-care. Train crew to recognize fatigue symptoms, manage rest, eat well, understand their body clocks.
This isn’t a luxury—it’s part of operational training.
And of course, leadership matters. If senior management doesn’t take this seriously, everything else becomes a formality.
Arturo (cont’d): One last point we must not forget: fatigue management doesn’t only apply to flight crews.
It must include all technical personnel—maintenance and flight dispatchers. Their jobs require intense concentration, and mistakes in either role can compromise a flight’s safety.

Arturo: To wrap up: flying fatigued is not a safe option. Period. It’s a hidden threat disguised as “normal.”
If safety is to remain our industry’s top value, we must stop treating fatigue as part of the job.
Nicolás: Absolutely. We need to change the narrative—from endurance to shared responsibility. That starts with a culture that protects the crew and supports riskbased decisions—not blind schedule adherence.
